**Some United HealthCare beneficiaries are delegated to the Southwestern Health Resources ACO (Accountable Care Organization). Christian Care House Calls is not in network with this ACO.
Please remember that it is the patient’s / patient’s family responsibility to keep the insurance information updated with Christian Care House Calls! If you or your loved one is in a facility, remember, the facility does not inform us when plans change!
If you or loved one has a supplemental plan along with Traditional Medicare, YOU must update Medicare with the supplemental plan information, for the claims automatically “crossover” to the supplemental policy. Failure to do so may mean you must pay the balance due and file the supplemental claim yourself.
